If you call us, we can take you through a series of questions to record your consent to be involved in the pilot. You can answer these same questions at the link provided in your letter or email.
We will ask you how you would like to be contacted for the assessment appointment and if you will have someone with you at the assessment.
You can decide to have a support person at the assessment for the whole time, or just for parts of the assessment.
We’ll also ask for your consent for this person to talk about your support needs. During the call, you need to tell us if you would like to do your assessment face to face or via video call, and if you will need an interpreter.
APM, Allied Care Group and HealthStrong are doing the independent assessments for this pilot.
Once we have this detail from you, we’ll ask APM, Allied Care Group and HealthStrong to contact you or your nominated person directly, to book your independent assessment appointment.
APM also deliver NDIS Local Area Coordination services. To avoid conflict of interest, APM can only provide the assessments in areas outside of where they deliver NDIS Local Area Coordination services.
Allied Care Group and HealthStrong are registered NDIS providers. They can only provide independent assessments to participants who do not receive their services as part of their NDIS plan.
The three organisations will deliver independent assessments for the pilot across metropolitan, regional and remote locations nationwide
