Survey open on Early Childhood provider report form

We are seeking feedback from providers about the Early Childhood provider report form (DOCX 96KB), which guides providers to write reports for us about the progress of participants aged under 7 years.

Providers can submit their feedback via the provider report form survey , before 5 pm AEST Friday 23 April 2021.

We launched the form in September 2020, to help providers write reports for the plan review process. We require reports ahead of a participant’s plan review. The form outlines the information we need, such as:

  • what services were provided, and by whom
  • a progress update including outcomes achieved, 
  • and future recommendations.

To date, use of the Early Childhood provider report form is highly recommended, but voluntary. 

In the Early Childhood Early Intervention (ECEI) Implementation Reset we gathered your feedback about our plans to improve the way we support young children and their families to reach their full potential. 

As part of the consultation we released a paper for families and carers, Supporting young children and their families early, to reach their full potential, outlining all our recommendations.

Recommendation 22 relates to the use of the Early Childhood provider report form.

The provider report form survey will contribute to feedback received from the reset consultation, assist us to make improvements and inform the use of this resource into the future.
